Multifocal Mastery: clariti® 1 day for Patients with Presbyopia

Today, advances in multifocal contact lens technology and design mean that your patients with presbyopia are no longer limited to spectacle lens dependency of readers and PALs or monovision CLs to regain their near vision.3 Modern multifocal contact lens designs offer an excellent opportunity for clear vision at all distances while maintaining stereopsis and offering patients the freedom from spectacle lens wear.

As presbyopia presents new vision challenges for those entering their forties and beyond due to presbyopia’s progressive nature,4 selecting a multifocal contact lens that addresses a patient’s vision needs is important in their wearing success.5

What has been your experience with the new clariti® 1 day multifocal 3 Add in practice?

Dr. Thornhill: I’ve fit clariti® 1 day multifocal 3 Add since June of this year and participated in CooperVision’s pre-launch in-market assessment of the lens. My experience with the new clariti® 1 day multifocal 3 Add has been nothing short of amazing and has been a game changer. CooperVision has simplified the fitting process with this lens with the fitting guide.6 If you have a good refraction and follow the fitting guide, it literally should not take more than two visits to finalize a fit.6 This includes the initial fit visit plus a follow-up if modifications are needed.

So far, I’ve introduced this lens to 70-plus patients. Using my refractions and the CooperVision fitting guide, I've only had to modify the final prescription on three out of 70 patients. 

What are your impressions of the new Binocular Progressive System™ in the new 3 Add design?

Dr. Thornhill: The Binocular Progressive System™ allows for exceptional distance and near vision at all stages of presbyopia.1 The design of the clariti® 1 day multifocal 3 Add gives a patient great clarity at all distances as they age. This is achieved by modifying the Add component of the lens on the non-dominant eye according to the fitting guide. In early presbyopes, the lens has a wide distance area that progresses to an aspheric low center-near Add design on both eyes. Aging presbyopes continue to keep clear distance vision with the wide distance area and low Add on the dominant eye. Clear intermediate and near vision is achieved as the central reading area is widened and strengthened in the medium and high Adds on the non-dominant eye.

What type of feedback are you receiving from your patients who are wearing clariti® 1 day multifocal 3 Add?

Dr. Thornhill: Some of the comments patients have made include:

  • Hands down, this is the best multifocal lens I have ever worn.” 
  • “Before wearing these contacts, I had to tote a pair of reading glasses around everywhere. I have kept my distance clarity, and now I don't need my readers to see my phone or computer screen.”7

Have you refit any established multifocal wearers into the 3 Add design? 

Dr. Thornhill: I’ve refit several patients from other multifocal lenses with relative ease. My approach is simple. At the end of an established wearer’s exam, I tell the patient, “There is a new daily multifocal contact lens out now. It’s working great. Would you mind trying it out for me and let me know what you think?” 

Nine times out of ten, they will agree to try the lenses. I then dispense a trial box of the appropriate lens for each eye and have the patient follow-up in a week. I also tell the patient about Binocular Vision System™ and instruct them to evaluate how the lenses work with both eyes open and not to try to compare the vision between the right and left eyes individually.1 I explain, “Your two eyes work together in these lenses to give you great distance and near vision.” I find this prevents unnecessary call backs. There is no need to make it hard. If you can get the patient to try the lenses, they will most likely be impressed with their overall vision.7 If a patient likes the contacts, they will order. 

Is the clariti® 1 day multifocal 3 Add a good lens for neophyte wearers and/or those who are new to presbyopia?

Dr. Thornhill: This is a great lens for both new and established contact lens wearers with presbyopia. I truly believe this lens provides the best overall distance and near vision of all the current multifocal contacts available. That may sound sales pitchy, but if a doctor fits enough of this lens using the fitting guide, they will agree.

Is there anything else that you’d like to add?

Dr. Thornhill: The clariti® 1 day multifocal 3 Add is the lens I wear now. I personally love this lens. I tell patients this is the lens I’m wearing and how well it works for me. This makes it easy to get my patients to try it as well.

Additionally, MyDay® multifocal contact lenses use the same design (Binocular Progressive System™) as the clariti® 1 day multifocal 3 Add.2 MyDay® multifocal is a great lens for patients who may have a steeper cornea or lens awareness in the clariti® lens.8
