Rebates can be completed online at our Rebates page or you can print the manual forms below for patients without access to the Internet.
Patients should submit a dated sales receipt, dated eye exam receipt, and original end panels from their boxes. Please visit our Rebates page if you have any other questions or contact your sales representative.
Please note: Rebate claims must be submitted within 60 days of lens purchase. All receipts must be from the same eye care practitioner who prescribe your contacts, or from a location affiliated with that practitioner.
