Educational Grants

APPLICATION PROCESS


To qualify for Grant support, all proposed Continuing Education Activities must meet the following criteria:

  1. Meet a legitimate clinical education or practice management objective or need
  2. Serve a broad audience of eye care practitioners or optometric organizations
  3. Be objective, balanced, and scientifically rigorous with evidence that the education supports a full range of treatment options
  4. Be developed independently of CooperVision employees or subsidiaries
  5. Demonstrate independent control by the conference organizers or program faculty regarding conference content, topics, faculty, invites, presentations, materials and educational methods

TYPES OF PROGRAMS/REQUESTS THAT CANNOT BE SUPPORTED THROUGH GRANTS


  1. Networking or referral meetings
  2. Practice development and/or staff training
  3. Marketing campaigns
  4. Patient education
  5. Vision screenings or health fairs
  6. Registration fees or travel to medical meetings
  7. Capital campaigns or building funds

SUBMITTING A GRANT REQUEST


The Grant funding application requires information for the applying organization, as well as details pertaining to the educational event/initiative itself. This includes, but is not limited to, the educational need for this program, its learning objectives and goals, evaluation methods for those objectives, and the types of support requested.

The following documents are needed to complete your application. Documents marked “required” will need to be submitted by all applicants.  As noted below, additional documents may be required for specific types of applications.

  1. Request Letter on Legal Letterhead
  2. Sample Invitation or Flyer (Required)
  3. Program or Activity Agenda (Required)
  4. Budget (Required for monetary requests)
  5. Completed W-9 (Required for monetary requests)
  6. Template of your organization’s standard Letter of Agreement for Commercial Support of COPE or State Board (if applicable)
  7. Evaluation or Assessment Tools (if applicable)
  8. 501c3 Letter of Determination (if applicable)
  9. ACCME, COPE or other Accreditation Certificates (if applicable)
