CooperVision Rebates

Follow these 3 simple steps to get a rebate:

Step 1
Visit your eye care practitioner (don't have one?) for a complete eye exam and to be fitted for your CooperVision contact lenses.
Step 2
Once you've purchased your eligible CooperVision contact lenses, you can begin the process of submitting your rebate.
Step 3
Mail in your completed form. Allow for 8 weeks to receive your rebate.

Please note: Rebate submissions must be postmarked within 60 days of lens purchase. Contact lenses must be purchased within 90 days of your eye exam, and all receipts must be from the same eye care practitioner who prescribed your contacts, or from a location affiliated with that practitioner. One rebate per person per twelve (12) month period and five rebates per address per twelve (12) month period. Monthly maintenance fee for rebate prepaid gift card waived for first 6 months after issuance. If a check is issued, it will expire and be null and void if not cashed within 60 days of issue.
