CLSA Continuing Education Rebate Program Enrollment Form

• CooperVision and the Contact Lens Society of America (CLSA) have agreed to cooperate in a program to promote contact lens educational programs offered by the CLSA for eye care practitioner's contact lens technicians and staff.
• Customers of CooperVision that sign up for this program will be eligible to earn product rebates, redeemable for CooperVision CLSA dollars, which may be used to pay for certain CLSA educational materials, membership fees, meeting registration fees, and meeting travel and lodging expenses.
• I agree to participate in the CLSA Continuing Education Rebate Program and agree to the CLSA Continuing Education Rebate Program Terms and Conditions.
